Schedule
Shift: 4x8, Days
Weekends: Every other weekend required
Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 72 hours per 2-week period
Requirements
- Minimum 1 year clinical experience
- Required: Acute care experience; Discharge planning / Care coordination
- Strong preference: Diverse case management experience (Med-Surg, prolonged LOS, ED)
- Required certifications: BLS; State licensure
- Charting: EPIC experience required
- Preferred experience: Med-Surg, long LOS case management, ED-case management
Roles & Responsibilities
- Provide comprehensive case management services to patients in acute care settings.
- Develop and implement discharge plans ensuring continuity of care post-discharge.
- Coordinate with interdisciplinary teams to facilitate a smooth transition of care.
- Document and maintain accurate patient records and case management activities.
- Provide referrals to appropriate home care, rehabilitation, or long-term care facilities as needed.
- Monitor patient progress and adjust care plans in collaboration with healthcare providers.
Compliance / Notes
- Orientation: 40 hours
- Floating: May be required to work across hospital areas.
- RTO (requested time off) must be included at the time of submittal.
- Background checks and licensure verification are required prior to submission.
