BJC HealthCare is seeking a compassionate and skilled Home Health Registered Nurse to deliver high-quality, patient-centered care in the comfort of patients’ homes. As part of our Healthcare & Medical Services team, you will play a critical role in supporting patients across the continuum of care, helping them manage complex health conditions, recover from illness or surgery, and maintain dignity and independence in their own environments. Working independently in the field while supported by a collaborative, interdisciplinary team, the Home Health Registered Nurse conducts comprehensive assessments, develops and implements individualized care plans, and provides direct skilled nursing interventions. You will perform thorough physical and psychosocial assessments, monitor changes in patient condition, and coordinate with physicians, therapists, social workers, and other care providers to ensure safe, seamless care. In this role, you will administer medications and treatments, including wound care, injections, and IV therapies, while closely monitoring patient responses and ensuring adherence to physician orders and evidence-based best practices. You will educate patients and family members on diagnoses, medications, equipment, and self-management strategies, empowering them to participate actively in their care and make informed decisions. A key focus of this position is chronic disease management for conditions such as heart failure, COPD, and diabetes, with an emphasis on preventing avoidable hospitalizations and improving overall outcomes. The Home Health Registered Nurse is responsible for accurate, timely documentation in the electronic health record, maintaining compliance with regulatory standards, payer requirements, and BJC HealthCare policies. You will assess the safety of each home environment, identify potential risks, and implement interventions to promote patient and caregiver safety, including infection prevention and safe mobility practices. Strong clinical judgment, critical thinking, and the ability to work autonomously in varied home settings are essential.
