Job Responsibilities Provide comprehensive physical therapy assessments and develop individualized treatment plans for patients in a long-term care setting. Implement and adjust therapeutic interventions to improve patient mobility, strength, and functional capabilities. Document patient progress, treatment outcomes, and any relevant clinical notes accurately in electronic medical records. Collaborate with interdisciplinary team members to coordinate patient care and ensure treatment continuity.
